6100016710 JK Request for Proposals – Health Services for Older Adults
The City of San Antonio (hereinafter referred to as “City”) is seeking proposals from Respondents (hereinafter referred to as “Respondent”) qualified to ESTABLISH AND OPERATE DEDICATED HEALTH SERVICES on a daily basis. These services will be offered by the Respondent AT NO COST to the City for all eligible participants in Senior Comprehensive Centers and Part-Time Centers located in City Council Districts 1, 2, 3, 4, 5, 6, 7, 8, 9 and 10. Services will include core health services, as well as proactively addressing chronic conditions and providing acute care to clients. These services will enhance the comprehensive social service delivery system already offered at the centers.
